Form 10 – Application for Amendment of Patent

Form 10 shall be filed when the owner or an applicant of a granted patent is deceased and should be substituted.

This form shall be filed upon such cases by the legal representative to the Controller after the death of an applicant, requesting the Controller to substitute the name of the Applicant deceased with his / her name.

Details required for this form:

1. Name of the applicant
2. Address, State and Nationality of the Applicant
3. Attachments such as death certificate and legal heir certificate
4. Address for service in India
