Form 15 has to be submitted by the applicant / applicants / legal representative of a lapsed patent which got lapsed due to non payment of the renewal fees within the period prescribed or extended. This form however should be submitted within 18 months from the date of on which the patent got lapsed.
In such cases the person applying for the same shall state the reason for the delay in payment of renewal fees, along with the evidence if any to the Controller along with the application. (Form 15)
