Form 8 – To be submitted upon filing a request or claim regarding mention of inventor as such in a patent.
This form shall be filed on the following cases:
- Applicant wishes to add the name of any person who is a participant in an invention and his name was not added to the list of Inventors
- Inventor wishes to remove his name voluntarily from the list of inventors of the patent
The details to be entered to the form are:
- State, Name, Address and Nationality of the Person (Applicant adding inventor / Inventor willing to get removed)
- Name, address and Nationality of the person to be added (mentioned/removed)
- Reason for such request.
- Address for service in India
